Chief Complaint
- Head pain – almost everyday, p/s 7-8
(forehead pain,temporal pain, migraine, cluster headache) - Neck and shoulder pain – every other day, p/s 7-8
- Sore Throat
Other Symptoms:
- Blurring of vision
- Pressure Behind the eyes
- Maxillary sinus headache
- Posterior back of the head headache
- Hair and scalp painful to touch, jaw deviates to one side when opening
- Bruxism
- Popping of jaw joints
- Lock jaw
- Vertigo
- Tightness of throat
- Voice fluctuations
- Stiffness
- Back pain
- Eye pain – p/s 5-6
- Ear pain – 2x a week, p/s 5
- Arm and finger numbness – everyday
